We hope you share our belief that regular preventive dental health care is a sound investment. In most cases we prefer to alternate your maintenance visits with your regular dentist. At the time of your maintenance visit, please advise any staff member of changes in address, telephone number, general health status, medications, or insurance coverage.
Fees and Payments
To help keep down the cost of your dental care, we ask that you pay at the time of your visit. If your treatment plan requires several visits, you will be given an estimate and asked to discuss financial arrangements with your treatment coordinator. We ask that whatever financial arrangements are agreed upon be adhered to. If unusual circumstances should make it impossible for you to meet agreed upon credit terms, please call or discuss the matter with your treatment coordinator. This will help us avoid misunderstanding and enable you to keep your account in good standing. Visa, MasterCard, Discover and Care Credit are accepted.
Monthly billing statements will be sent to remind you of payment due and/or update you on the status of your account if dental insurance is present.
As a courtesy to you, our office will gladly file your dental insurance. You will need to supply us with identifying information (name, group number, social security number, etc.) and provide signatures where indicated. Insurance companies do not attempt to cover all dental costs. It will be your responsibility to pay any deductible or any other balance not paid by your insurance company.
We know questions can arise on insurance matters. We encourage you to discuss such questions with our business office staff. We will be happy to help you receive the maximum benefits; however, the agreement of the insurance company to pay for your dental care is a contract between you and your insurance company.
